*Duties:*
- Provide comprehensive and coordinated care to patients in a hospital or managed care setting.
- Conduct diagnostic evaluations and assessments to determine the healthcare needs of patients.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Coordinate and monitor patient care, ensuring appropriate utilization of resources.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure quality patient care.
- Conduct regular patient assessments and reassessments to evaluate progress and adjust care plans as needed.
- Provide education and support to patients and their families regarding their healthcare needs and treatment options.
- Maintain accurate and up-to-date medical records, documenting all patient interactions and interventions.
- Participate in utilization management activities to optimize patient outcomes and resource utilization.
*\*$2,500 sign-on bonus\**
*Qualifications:*
- Valid Registered Nurse (RN) license in New Hampshire
- Bachelor's degree in Nursing (BSN) preferred.
- Minimum of 3 years of experience in case management or a related field.
- Strong knowledge of medical terminology, disease processes, and treatment modalities.
- Excellent communication and interpersonal skills, with the ability to effectively collaborate with multidisciplinary teams.
- Proficient in utilizing electronic medical records systems for documentation and care coordination.
- Ability to prioritize tasks, manage time effectively, and work independently with minimal supervision.
- Knowledge of utilization management principles and practices is highly desirable.
- Experience in primary care or a hospital setting is preferred.
