Licensed Practical Nurse - LPN 13 Week Contract to Hire Prefered!

LPN needed for Residential Homes - FULL TIME! 13 Week CONTRACT TO HIRE Preferred!

Location: Evergreen Park

Shift Details: Full Time, 8am-4:30 (Flexible Start), 40 hours per week

Pay Rate: $33/hr

Requirements:

• CPR

• Diploma/License

• 2 STEP within the last year

• TB MMR/ VARICELLA/ TDAP

• PHYSICAL Within the last year

• Proof of COVID VACCINATION

• Experience working with the developmental disabilities population

Essential Responsibilities:

• Perform onsite client assessments and training, as required.

• Document specific client medical needs and interventions to include client progress notes, as needed.

• Monitor aspects of client medication regimes; review medication records, orders and bubble packs; review MARs.

• Review client medical records on a monthly basis or more frequently, if necessary.

• Develop interventions designed to allow staff to meet the specific needs of the clients.

• Communicate with Program Directors, CLS Administrators, Division Director, Client's Physicians and other medical personnel regularly.

• Attend client staffing's and present nursing notes, as requested.

• Conduct staff training and skills observations on an on- going basis.

• Assist in the facilitation of the client's hospitalization via admission, discharge/after care.

• Planning with visits to the hospital as requested.
