Start Date: ASAP (90 days for full privileges)

Contract Length: 6 months, ongoing

Schedule: Monday - Friday, 8a-5p, no call

Details:

• Locums provider must be Board Certified OBGYN (MD or DO).
• Schedule is Monday - Friday, 8a-5p
• No Call, OT pay available for patient care after shift.
• Must have an active IL license and DEA/CSR
• Level 1 Trauma Center
• EMR is EPIC
• 100% Female, all ages except peds
• Procedures required: Colpo, LEEP in office, not planning for procedures outside of the office at this point. Surgical patients will be referred to perm providers. No case volume required for clinic.
• Cases per day: 16-20
• Support Staff: CMA, RN

Credentials: Active IL Medical License, BC in OBGYN, active DEA & CS licenses
