Nurse Practitioner - Palliative Care
Please note this position is primarily outpatient, but will require occasional inpatient call and weekend coverage.
JOB DESCRIPTION:
The Palliative Nurse Practitioner provides expertise in a consultative role to patients and families with advanced disease requiring palliative services. Works in collaboration with a team of palliative care physicians and nurse practitioners. Collaborates with attending physicians, specialists and non-physician providers as applicable for the purpose of coordination of care to effectively meet the medical, psychological and spiritual needs of the patient. This enables patients to live life as fully and comfortably as possible in an atmosphere where patients and families are able to share their thoughts, hopes and concerns.
EDUCATION:
Registered nurse who has successfully completed formal education program approved by the Vermont Board of Nursing, or state or national accrediting agency which prepares nurses to function in advanced nursing practice. Must also be certified by national certifying agency recognized by the Vermont State Board of Nursing.
EXPERIENCE:
Family NP is preferred. Palliative Care Board certified or eligible preferred. Eligibility or certification must be obtained within one year.
